Shalom u'Vrachah!
In response to your letter from the third of Va'eschanan, with the attached note to be read at an auspicious time at the holy resting place of my father-in-law, the Rebbe zatzal. Also, regarding his responsa, which as you requested is enclosed here.
We have only the directive of my father-in-law the Rebbe, the tzt"z in his responsa Even HaEzer at the end of section 29 (part I), who is lenient in such cases since it concerns health—especially in your case, as you wrote that doctors believe each pregnancy puts her in danger.
This ruling should especially be emphasized in our generation, this orphaned generation. Even those who are stringent do so because of the seriousness of shichvas zera l'vatalah (wasting seed), but being stringent here often leads to extended abstinence and thereby causes more frequent occurrences of shichvas zera l'vatalah than using a moch (barrier), since then it depends on both spouses and there is no difference regarding days of purity between them, etc., until ultimately these stringencies cause the opposite of their intention—enough said.
Even though there is room for doubt whether this should be publicized among unlearned people—so that immodest women do not learn from modest ones and healthy women from weak ones—those who ask should be instructed according to the opinion of my father-in-law the Rebbe tzt"z.
With blessing.
P.S. Enclosed is the booklet "Tzion B'Mishpat Tipadeh" which you published; surely you will merit to benefit many with its content and the merit of the many depends on you.
The opinions regarding using a moch—in addition to those cited in tzt"z's responsa there—see She'elot U'Teshuvot Chaim Assefat Dinim Ma'arachot Ishut 1:32 and Pe'at HaSadeh there Ma'arachot Ishut sec. 1 (who ruled practically to permit). What you brought from Maharsham I:158 that near immersion or near her period one should not have relations since most women conceive then—as opposed to other times—and as written in Shulchan Aruch Orach Chaim sec. 615—I do not have that responsa at hand to check inside. But with all due respect, such a novel restriction is very puzzling since none of the early or later authorities made such a limitation. In my humble opinion, as above, stringency in this case risks causing exactly what we wish to avoid. We must not add stringencies on our own authority to rulings from those whose teachings we rely on.

Regarding your comment about the severity of this sin and its rectification—see Tanya end ch. 7 and also Iggeret HaTeshuvah chs. 3 and 9.
Summary
The Rebbe writes that when pregnancy poses a danger to a woman’s health, one should follow the lenient ruling of the Tzemach Tzedek permitting birth control. He warns against unnecessary stringency that could lead to greater halachic problems and emphasizes instructing questioners accordingly.
