Shalom u'Vrachah!

So finally—after his long silence—his letter from the 15th of Tammuz was received, in which he writes about his visit to the doctor and what he was told. It is well known how precious the health of a Jewish body is in the eyes of Hashem. As we heard from my revered father-in-law, the Rebbe zatzal nishmaso Eden zichrono tzaddik livrachah zechuso yagen aleinu amen (may his memory be a blessing), that once his father, the Rebbe, revealed himself to him and said: See how great is the value of the body for which so much Torah and mitzvos have been poured out. Therefore, it is obvious that one must guard his bodily health. Since this is among those matters that Torah entrusted to doctors to instruct regarding eating and drinking routines etc., one must follow their instructions.

It is not understood what you write—that because of this you cannot engage in avodah of prayer. If your intention is that you do not eat or taste before prayer—know that it is well established among later authorities and so ruled by our holy leaders that where bodily health requires it, it is permitted to taste before prayer; indeed, it is a mitzvah and an obligation—especially according to the saying of the tzt"z: It is better to eat for the sake of davening than to daven for the sake of eating. Regarding details of eating, discuss with your mashpia and act according to his instruction and as per doctors' orders.

As for what you write about nerves—surely this too can be easily improved by taking walks from time to time and similar things. Most importantly—through complete trust in Hashem who leads each one of us (and you are included) on a path ascending toward Beis El (the House of G‑d). One must strengthen bonds with Hashem and His Torah through love of Hashem, love of Torah, and love of Israel. All this relates to melancholy—which is advice from the yetzer hara to distract one from Torah and service—and especially from 'Bechol Derachecha Da'eihu' (In all your ways know Him).

Awaiting good news and blessing you with success and proper health in all respects together.

Summary

The Rebbe teaches that when health requires eating before prayer, it becomes not only permissible but a mitzvah—especially as taught by the tzt"z. Following medical advice while strengthening faith and connection to Torah brings both physical and spiritual well-being.
