במענה על מכתבו, בו מודיע אשר לומד בהישיבה בספריא זה שנתיים וב' חדשים, ויש רופא מייעץ לעשות נתוח בהשקדים בגרון ומיעצו לעשות עתה וכן מבקש ברכה בעד אביו מוה"ר... שי'
הנה א) בנוגע להנתוח, ישאל עוד רופא אחד מומחה במקצוע זה, ואם גם הוא יסכים לעשות הנתוח ועתה, הרי יהי רצון שיהי' הנתוח כשורה ובשעה טובה ומוצלחת, ובהש"ק שלפני הנתוח יאמר את כל התהלים - ואם באפשר בלי הפסק, היינו גם בלא אמירת יהי רצון כ"א אחר כל הספרים - וכשידע ברור יום הנתוח יודיע לכאן.
ב) את אביו שי' אזכירו בהיותי על הציון של כ"ק מו"ח אדמו"ר זצוקללה"ה נבג"מ זי"ע, וימסור לאביו שי' שישמור את שלשת השיעורים השוים לכל נפש כתקנת כ"ק מו"ח אדמו"ר, והם בחומש תהלים ותניא.
בטח במכתבו השני יודיע בפרטיות ע"ד למודו והנהגתו הטובה ביראת שמים.

In response to your letter in which you inform that you have been studying in the yeshivah in Sepharia for two years and two months, and that a doctor advises a tonsil operation in your throat and recommends doing it now; you also request a blessing for your father Moreinu HaRav... shiyichyeh: a) Regarding the operation—consult another expert doctor in this field. If he too agrees that the operation should be done now, then may it be G‑d’s will that the procedure go smoothly and at an auspicious and successful time. On the Shabbat before the operation, recite all of Tehillim—and if possible without interruption (that is, even without saying "Yehi Ratzon" except after each book). When you know clearly the day of the operation, notify here.
b) For your father shiyichyeh—I will mention him when I am at the resting place of my revered father-in-law, Admor zatzal nishmaso Eden. Convey to your father shiyichyeh that he should observe the three daily study portions established by my revered father-in-law: Chumash, Tehillim, and Tanya. Certainly in your next letter you will report details about your studies and good conduct in fear of Heaven.
