On what to disclose in a shidduch, and when, the Rebbe's guidance:

Timing: sensitive personal matters should be disclosed after the first meeting, once both sides want to go further (IK 10821).

A specific question about what must be disclosed should be brought to a competent rav, whether it concerns a health matter, an adoption, or a past relationship (IK 5681, 5762, 10085).

Past health issues need not stand in the way of a match when there is evidence that all is well, such as healthy children in the family. Seek advice from trusted friends (IK 3983).
