In response to your message regarding your wife's condition, may the Holy One fulfill her pregnancy term properly and easily, and may she give birth to a healthy child at the right time and in good health and ease.
In response to your questions: a) It is obvious that the longer you extend your Torah study in kollel (whether in your current location or abroad—whichever is easier for the in-laws to agree upon), the better. As our master the Alter Rebbe writes in Hilchos Talmud Torah: two or three years from the time of marriage.
b) Even after this period—there must be fixed times for Torah study with a partner; therefore, you should live in a city where this is possible.
c) Regarding livelihood—consult with a knowledgeable friend experienced in such matters.
