Accordingly, you may go on Rosh Chodesh, and after immersing in a mikveh on that day. However, beforehand you should add something in honor of the departed (such as repairing the gravestone if necessary). Most importantly: arrange with an institution (such as a Chabad kolel or similar) that they will observe her yahrzeit, etc. On the day of your visit, give charity before and after going.

Summary
The Rebbe teaches that visiting a parent's grave after many years is permitted when proper customs are observed: immersion in a mikveh, acts of honor for the deceased, arranging Torah study in their merit, and giving charity before and after the visit.
