אם יש לאדם אשה ומתה לא ישא אחרת ששמה כשם הראשונה, או אם מת בעל לאשה לא תקח בעל ששמו כשם הראשון, כי יחשבו אחר הראשון, ואם יהי' הדבר כך לעולם לא יהי' בטוב או ימות אחד מהרה.

1) Regarding a man marrying a woman with the same name as his first wife—see what is brought in Shut She'elat HaEzra Siman Alef, Section 12 (and in Sefer Chochmat HaNefesh). Regarding a woman whose second husband's name is like her first's—I have not found any clear source on the names of the second husband and the first. In general, there is a distinction between men and women according to Mishnah Berurah (Orach Chaim Siman Resh Mem, Even HaEzer Siman Kuf Hei) where it is explained—only regarding the man's intention (even though incidentally—not for halachic ruling—the woman's intention is also mentioned (Pesachim 112a, Section Alef)). Also, since the reason (that the name arouses thought) is not even mentioned regarding a man—in relation to the names of bride and mother-in-law—it seems we do not worry about this. Nevertheless, if it is possible to change the name—it is better, etc.
2) Regarding saying Kaddish Yatom after Kiddush Levanah—according to Shulchan Aruch (Sha'ar HaOtiyot page 74b), the conclusion of Kiddush Levanah follows Abaye's opinion and therefore—Kaddish DeRabbanan. And so it is in Seder Yaavetz. For us, who conclude (according to Seder HaAriZal and others) with Aleinu—it is obvious that one says Kaddish Yatom. So too in Pe'at HaShulchan in Seder Kol Yaakov. And more.
3) I will mention again regarding your request for content of your letter. Since due to preoccupations etc.—writing an organized letter was delayed, I requested to call you and ask your forgiveness for this. After writing the above I saw additional warnings* in manuscripts—in Tzava'at Rabbi Yehudah HaChasid [printed in Sefer Chassidim—Jerusalem 5717 p. 30] as follows: If a man has a wife and she dies, he should not marry another whose name is like the first; or if a woman's husband dies she should not marry another whose name is like her first husband, because they will be reminded of the first one, and if this happens things will never go well or one may die quickly. And in the publisher's note there: see Sefer Chochmat HaNefesh by Rokeach p. 73b.
