As for what you wrote about your doubt concerning the name—where the name given at birth was one way and at school she was called by another name—it is surprising that there is any doubt here, since the laws have been decided according to the details of these matters in Hilchos Shemos (laws of names) relevant to marriage contracts, ketubos, etc.
