במענה על מכתבו מד' תמוז והקודם לו, בו שואל חוו"ד במה שהוסיפו לפלונית שם מחמת חולי ואח"כ נזכרו שהשם שהוסיפו הוא גם שם אחותה.
ולפלא שאינו מזכיר אם כבר עברו שלשים יום שהחזיקו בשם הנוסף, אף כי בכל אופן לדעתי אין לעשות מילתא דתמיהא, וכמבואר בכ"מ ובפרט בצוואת רבי יהודא החסיד, ולכן אם אפילו עברו שלשים יום הרי ע"פ המבואר להלכה למעשה בשם שנשתקע, יש להוסיף לה שם חדש וכמנהג ישראל במי שברך בעת שאחד מקרובי' יעלה לתורה ולהחזיק בו, וכבר מבואר בכ"מ בשם הנוסף שהוא בא לראשונה, ויה"ר ששינוי השם ישנה המזל לטובה ולברכה בטוב הנראה והנגלה.

In response to your letter from the 4th of Tammuz and the previous one, in which you ask for my opinion regarding adding a name to a woman due to illness, and then realizing that the added name is also her sister's name.
It is surprising that you do not mention whether thirty days have already passed since the additional name was used. However, in any case, in my opinion one should not do something odd or questionable, as explained in many places and especially in the will of Rabbi Yehudah HaChassid.
Therefore, even if thirty days have passed, according to what is explained in practical halachah regarding a forgotten name, one should add for her a new name according to Jewish custom during the Mi Shebeirach when one of her relatives is called up to the Torah and maintain it. It is already explained in many places that the additional name is considered as if it was given for the first time.
May it be G‑d's will that changing the name will change her fortune for good and for blessing in an obvious and revealed way.
