הכתוב במכתבי הקודם, בהחילוק שע"פ שו"ע וכו' לא היתה כוונתי לשלול לגמרי נתוח הפראסטאטא, באם רופא מומחה מייעץ לעשות אותו, כי אם להורות על חילוק אופני הנתוח בזה, אשר ישנם רופאים שבכלל אין מנתקים את השביל.
ועל זה הבאתי המועתק באוצר הפוסקים חלק א' [מהדורא שני' עם הוספות, ירושלים תשט"ו].
ב)אפילו מהמנתקים - כמה עושים זה לאחרי שנעשה חלק העיקרי של הנתוח, שאז כבר יש צורך בפועל [לדעתם, עכ"פ] לשמירה מסיבוכים, משא"כ כשמתחילים בנתוק.

כן להוסיף אשר ישנם נתוחים דכיס השתן שמפני אותו הטעם דלהשמר מסיבוכים מפסיקים ומנתקים כו' בסמיכות למקום הניתוח ולפעמים גם במקומות שיש לדון בזה עפ"י השו"ע, שלכן העירותי על זה, אף שבמכתב כת"ר הא' לא היה נזכר אלא הנתוח האחרון, ועל כת"ר הסליחה על ההערה עוה"פ.

In response to your letter from Sunday. What was written in my previous letter, regarding the distinction according to Shulchan Aruch etc., my intention was not to completely prohibit prostate surgery if a specialist doctor advises it, but rather to point out the differences in surgical approaches here, as there are doctors who do not sever the duct at all. For this I cited what is quoted in Otzar HaPoskim vol. I [second edition with additions, Jerusalem 5715].
Even among those who do sever — many do so only after the main part of the surgery has been completed, when there is already an actual need [in their opinion, at least] for protection from complications, as opposed to beginning with severing. And this is clear.
I should also add that there are surgeries of the bladder where, for the same reason of avoiding complications, they interrupt and sever etc. close to the site of surgery and sometimes even in places that are subject to halachic discussion according to Shulchan Aruch; therefore I pointed this out, even though in your first letter only the latter surgery was mentioned. And please forgive me again for this comment.
