ע"י הוו"ח אי"א נו"נ מו"ה יהודה יוחנן שי' ספרנאי נמסרה לי בקשתו לרפואה קרובה ולבריאות, ובעת רצון יזכירוהו על הציון הק' של כ"ק מו"ח אדמו"ר זצוקללה"ה נבג"מ זי"ע נשיא ישראל. וכיון שברכה היא כמו גשם, וכלשון רז"ל גשמי ברכה, הרי עליו מצדו להכין את עצמו להיות כלי לקבלה, והוא על ידי הוספה בלימוד תורתנו תורת חיים וקיום מצותי' והשפעה גם על הסביבה בכיוון זה. והנה לכל דבר צריכה להיות אחיזה גם בדרכי הטבע, ולכן ישאל מחדש חוות דעת רופא מומחה במקצוע זה וכהוראתו יעשה, והרופא כל בשר ומפליא לעשות יתן להרופא הרעיון הנכון איך לייעצו. ואחכה לבשורה טובה בכל האמור לעיל.
מהנכון הוא שהי' שיבדקו את התפילין שלו וכן של אביו - שיחיו.

Through the vavac"h iy"a nu"n Moh"e Yehudah Yochanan Sapernai your request was conveyed to me for a speedy recovery and health. At an auspicious time you will be mentioned at the holy resting place of my father-in-law, the Rebbe zatzukal lehe nbgm zya, leader of Israel.
Since a blessing is like rain—as our Sages say "gishmei berachah" (rains of blessing)—one must prepare oneself to be a vessel to receive it. This is accomplished by increasing in the study of our Torah of life and fulfilling its mitzvos, as well as influencing one's surroundings in this direction.
Everything must also have a basis in natural means; therefore, consult anew with an expert doctor in this field and follow his guidance. May the Healer of all flesh Who works wonders grant the doctor the right idea on how to advise you.
I await good news regarding all of the above.
It is proper that your tefillin and those of your father be checked.

Summary
The Rebbe teaches that blessings require preparation—just as rain needs vessels to be collected. One should increase Torah study and mitzvah observance, positively influence others, seek expert medical advice, and ensure tefillin are kosher to fully receive G‑d's blessings.
