Shalom u'Vrachah!
The telegram and your letters (including from the 22nd of Teves, etc.) have been received. Regarding the couple you mentioned—act according to the advice of a trusted doctor who has no personal interest in the matter.
I was greatly astonished to read in your letter the reasoning of so-and-so regarding his difficulty with donning tefillin—that it is forbidden for a person to harm living creatures. What relevance does this have to our case?! After all, tefillin are made (also) from the hides of animals that died naturally (see Shabbos 108, section 1).
On the contrary, according to his logic—instead of letting the animal hide rot and be lost or making it into shoes or other utensils for human use or similar—he has the mitzvah to make from the hide something that will elevate a person and sanctify him, even adorning his head as Scripture says.
Especially in light of what is explained in Kabbalah and Chassidus regarding what is accomplished through the mitzvah of tefillin in all worlds (see Kuntres Acharon Daled Hei David Zmiros Kuf Lamed and elsewhere).
Summary
The Rebbe teaches that using animal hides for tefillin not only avoids harm but fulfills a higher purpose by elevating physical matter through mitzvos. This act brings holiness into all realms, as explained in Kabbalah and Chassidus.
