במ"ש אודות התפלה שלאחר הלימוד שנדפס בסידורים - לא ראיתי בתוך אנ"ש שיאמרוה.
בהנוגע לאמירת תחנון בתפלת מנחה, מפורסם מנהגנו לאומרו (לבד, כמובן, באותם הימים המצוינים בסידור מאדה"ז לפני ובא לציון), וע"ד ששמעתי פעם מכ"ק מו"ח אדמו"ר זצוקללה"ה נבג"מ זי"ע בהנוגע ליום היאצ"ט שמנהגנו לומר תחנון, שאמר הצמח צדק: ווען איז נאך אזא צייט אויף אויסצובעטין. שעד"ז הוא גם בתפלת מנחה שאמרו רז"ל לעולם יהא אדם זהיר בתפלת המנחה שהרי אליהו לא נענה אלא בתפלת המנחה.
בהנוגע להשם שנוסף - יבררו אצל רב מורה הוראה בסביבתו.

We acknowledge receipt of your letter from the month of Adar. May it be G‑d's will that from now on you and every single member of your family will know no sorrow or distress, but only goodness and kindness all your days.
It is obvious and self-understood that matters of livelihood will be arranged properly, in a manner of tranquility and honor, and that you will share good news regarding yourself and every member of your family in revealed and apparent good.

P.S. Regarding what you wrote about the prayer after learning that is printed in the Siddurim—I have not seen among Anash that they say it. Concerning saying Tachanun at Minchah: our custom is well-known to say it (except, obviously, on those days specified in the Siddur of the Alter Rebbe before U'va L'Tzion). And as I once heard from my revered father-in-law the Rebbe zatzal regarding a yahrtzeit day—that our custom is to say Tachanun—he said that the tzt"z commented: "When else is there such an opportunity to plead?" Similarly with Minchah prayer: our Sages said one should always be careful with Minchah prayer, for Eliyahu was only answered at Minchah.
Regarding the added name—consult a Rav who is a halachic authority in your area. The customs for the twelve months as well as for yahrtzeit are printed in the booklets for Yud–Yud-Gimmel Shevat and Beis Nissan.
