לפי דעת המומחים הכי גדולים במקצוע זה אשר כאן, ועל יסוד כמה וכמה מקרים שטיפלו בהם, הנה אם המחלה היא ממושכה כמה שנים וכל הטיפולים כבר עשו ולא הועילו, אז דעתם נוטה לניתוח.

ומובן שעל כבודו להחליט אם המקרה בנידון דידן הוא מסוג הנ"ל, כיון שהחולה נמצאת בטיפול המוסד שבהנהלתו.

מתאים לציווי חכמינו ז"ל והייתם נקיים מה' ומישראל הנני עונה גם על ענין השני אשר במכתבו וסגנונו המכוון אלי, שמתפלא איך זה אני רוצה להשפיע מעבר לאוקינוס וכו' ומסיים אשר "ישראל אמנם ארץ חדשה, אבל כו'".

והנה פשיטא שאין רצוני לפגוע בכבוד ארץ ישראל או בכבוד אישי של מישהו, ובפרט באלה שאינם מוכרים לי. אבל בהנוגע לטיפול לובוטומי שנתחדש בשנים האחרונות, הרי ככל ענין מדעי, ובפרט ככל ענין של טיפול רפואי, אי אפשר שמשנה לשנה לא יהיו שינויים ביחס הרופאים לטיפול זה וגם באופן ביצועו וככל שיגדל מספר המקרים הנבחנים יהיו שנויים אלו מבוססים יותר, ועל פי ההשערה, קיימים לזמן ארוך יותר. ובודאי לכ' ידוע כל זה - הרבה יותר מאשר לי.

על פי האמור, מובן שבכל טיפול שאי אפשר לתקנו אח"כ, וכמו ניתוח, הרי, כלשונו גם במכתבו, יש להשתמש בו רק אחרי כל נסיון אחר, זאת אומרת רק במקרה שמוכרחים לעשותו ואי אפשר לחכות אפילו באופן דשב ואל תעשה. ולכן ראינו אשר גם בארצות הברית ממעטים בניתוח זה, וגם אז - רק לאחרי כמה שנים של טיפול בחולה, וברוב המכריע שעושים ניתוח לובוטומי - הוא מפני שלא היתה אפשרות של השגחה מעולה (מחוסר אמצעים כספיים). ולאחר כל זה, וגם זה בודאי ידוע לכבודו יותר מאשר לי, אם גם הניתוח מצליח, הנה אף שכבודו מזכיר במכתבו "שכמעט כולם חזרו לחיים תקינים" מקרים כאלו שיחזרו לחיים תקינים כאן - נדירים ביותר וביותר, אם בכלל היו מקרים כאלו.

ולהשתמש בסגנונו של כבודו שאינו "רוצה להיות שותף לרצח", הרי בניתוח האמור אין זה ניתוח רק בגוף, כי אם ניתוח גם בחיים הנפשיים של המנותח, ואם תמצא לומר, הרי זה פוגע בחלק מחייו הנפשיים - ז.א. שיש בזה ג"כ רצח חלקי עכ"פ, אלא שבלית ברירה - בוחרים את הרע במיעוטו.

כל הנ"ל מבאר עמדתי בנוגע לטיפול האמור, מבוססת על הוראות חכמינו ז"ל בתורתנו תורת חיים, שאפילו הצלה לשעה ג"כ יש להשתדל בה ככל האפשר, ובפרט במקרה של חולי נפש, אשר בזה כמה ענינים עדיין אין ברורים הן בהנוגע לגורמי המחלה והן באופן רפואתה, וביותר בשנים האחרונות, אשר טיפולים עקריים ושנויים עקריים מתחדשים בשטח זה לעתים הכי תכופות. ולכן כדאית כל השתדלות לדחות טיפול הלובוטומי' למשך זמן איזה שיהי' - עד שאין ברירה בהחלט.

את כבודו הסליחה על כתבי אליו בשטח שהוא שלו. ויהי רצון אשר הרופא כל בשר ומפליא לעשות יצליחו במילוי תפקידו במילואו. וזכות הרבים מסייעתו.

מצב החולה: דלעיל אגרת ו'שנח.

רוצה להשפיע: אם לעשות נתוח.

אבל: לא יחשוב שאנחנו מפגרים ביותר בטיפול רפואי.

According to the opinion of the greatest experts in this field here, and based on several cases they have treated, if the illness has lasted for several years and all treatments have already been tried without success, then their opinion leans toward surgery. Of course, it is up to you to decide if the case at hand fits this description since the patient is under the care of your institution.

Respectfully and with blessings. In accordance with our Sages' directive "VeHeyitem Neki'im MeHashem U'MiYisrael" ("And you shall be clean before Hashem and Israel"), I am also responding to the second matter in your letter and its tone directed at me. You wonder how I wish to influence from across the ocean and conclude that "Israel is indeed a new land, but..."

It is obvious that I do not intend to offend the honor of Eretz Yisrael or anyone personally—especially those whom I do not know. But regarding lobotomy treatment which has emerged in recent years: like any scientific matter—and especially any medical treatment—it is impossible that from year to year there are not changes in doctors' attitudes toward this treatment as well as in its implementation. As more cases are examined these changes become more established and presumably last longer. Certainly you know all this—much more than I do.

Based on this, it is clear that for any treatment which cannot later be reversed—such as surgery—as you yourself wrote in your letter, it should only be used after every other attempt has failed; that is, only when it is absolutely necessary and one cannot even wait by doing nothing. Therefore we see that even in the United States this surgery is rarely performed—and even then only after several years of treating the patient—and in most cases where lobotomy is performed it is because there was no possibility for excellent supervision (due to lack of financial means).

After all this—and again you certainly know this better than I do—even if the surgery succeeds, although you mention in your letter "that almost all returned to normal lives," such cases here where patients return to normal lives are extremely rare if they exist at all.

To use your own phrase about not wanting "to be a partner to murder": with this surgery it is not just an operation on the body but also on the patient's mental life; if one wishes to say so—it harms part of their mental life—in other words there is also partial murder here at least; only when there is no choice does one choose the lesser evil.

All of the above explains my position regarding this treatment—based on our Sages' instructions in our Torah of Life—that even saving a life temporarily must be pursued as much as possible; especially with mental illness where many aspects remain unclear both regarding causes and cures—and especially in recent years when major treatments and changes emerge very frequently in this field.

Therefore every effort should be made to postpone lobotomy treatment for as long as possible—until there is absolutely no alternative.

Please forgive me for writing to you about a field that is yours. May it be G‑d's will that He Who heals all flesh and performs wonders will grant you success in fulfilling your role completely. The merit of serving many will assist you.
