Regarding what you wrote about the name—it is obvious that there is absolutely no reason for doubt or hesitation. For the Arizal has informed us that "the Holy One blessed be He places in the mouth (of the father and mother) that very name which is necessary for that soul... and this name is inscribed above in the Heavenly Ledger" (Shaar HaGilgulim HaKadosh 23).
About the name: There were people who said I should give after the grandfather.
