במענה למכתבה - בנוגע לנוסח המצבה שכותבת, לדעתי נכון להזכיר שם האב - דודי - בעל הנפטרת ז"ל (בהוספה שהוא מגזע אדמו"ר הזקן וכו'), וכמובן שם אבי' (הררד"צ ז"ל).
הנוסח בפרטיות - יש להתייעץ עם רבני אנ"ש שי' על אתר - כיון שבכגון זה צריך להתחשב עם הנהוג בבית-החיים זה וצ"ל הוראת הרב במקום.
בקשתי את ידידי הרה"ח וכו' הרב אברהם שי' לשתפני בהוצאות האישפוז בביה"ח וכו'.
מברקי והמכתב הקודם בטח נתקבלו בעתם.

In response to your letter regarding the wording of the matzeivah you write about, in my opinion it is proper to mention the father's name—my uncle—the husband of the deceased of blessed memory (adding that he is a descendant of Admor HaZaken, etc.), and of course her own father's name (Hararad"tz of blessed memory). For the exact wording, you should consult with the local Anash rabbanim on site, since in such matters one must take into account the customs at that cemetery and there must be guidance from the local rav.
