במענה למכתבו, וכבקשתו בא המענה מבלי לחכות על התור.
בו כותב בשאלה אודות אשה, ששני פעמים כבר עשו לה נתוח קסרי (לידה באופן דיוצא דופן) וכיון שהיא עומדת עתה ללדת, מציע הרופא באם שגם בפעם הזאת, תהי' הלידה באופן האמור, אז לדעתו יש חשש באם תתעבר עוד הפעם, ולכן רוצה לעשות לאחרי הלידה, מעין נתוח באופן שלא תוכל להתעבר עוד.
והנה בכלל אין עניני לפסוק דינים, אבל מה שלכאורה פלא בהאמור, שהרי ידוע דעת הרופאים (דוקא בשנים האחרונות), שגם לאחרי לידה באופן יוצא דופן, אפשרית לידה באופן נורמלי, כן לדעתם אין הגבלה מצד הבריאות, במספר לידות באופן יוצא דופן, כי אם תלוי במצב בריאות הכללי של האשה בעת הלידה, ז.א. שאין להחליט בהנוגע לחשש עבור ולידה כ"א בהתאם למצב בריאותו אז, ובזה הרי כמה תרופות להחזקת הבריאות בכלל, וגם לחיזוק האם וכיו"ב.
עוד זאת, ידוע פסק דין אדמו"ר הצמח צדק, שבמקום חשש בעיבור כנ"ל מתיר לשמש במוך, וע"פ הטעמים נראה שזה קל יותר מאשר ע"י נתוח ששוללים לגמרי אפשרית עבור.
ועוד להוסיף (ובטח גם זה יסבירו הרופא), כי על פי רוב, הנתוח למניעת עבור, נעשה באופן שהם קושרים (וסותמים) את הצנורות אשר באשה, ולפעמים קרובות, קורה, אשר קשירה זו מתפרדת, ואפשרית העבור חוזרת לקדמותה אלא שאין יודעים עד"ז, במלות אחרות, מצד המטרה להזהר מעבור, צריך עיון מה טוב יותר, האם באופן דפסק הצמח צדק הנ"ל או באופן דקישור הצנורות הנ"ל.
ועוד נקודה להוסיף וגם היא עיקר, שבכלל על פי החקירה ודרישה בשטח חכמת הרפואה בשנים האחרונות, עלול כל יום ויום לחידושים ותגליות שלכן על האדם להמנע לעשות ענינים כאלה, שאי אפשר לשנותם לאחרי כן, אפילו באם יתחדש טפול השולל הכרח הענין שעשו.
כיון שכותב אשר הרופא הנ"ל הוא שומר תורה ומצות, נוסף על היותו מומחה בהענין, מהנכון אשר לאחרי שיברר נקודות האמורות אצל רופא מומחה אחר באופן פרינציפיוני, ידבר עוד הפעם עם הרופא השומר תומ"צ ויברר כל הנ"ל.
והשי"ת המשגיח על כאו"א בהשגחה פרטית, יראם הדרך הנכונה איך להתנהג בהאמור.
ואכפול ברכתי אשר השי"ת ימלא ימי הריונה כשורה ובנקל ותלד זחו"ק בעתה ובזמנה כשורה ובקל.

In response to your letter, and as you requested, I am replying without waiting for your turn. You write with a question regarding a woman who has already had two cesarean sections (a birth in an unusual manner), and since she is now about to give birth again, the doctor suggests that if this birth is also by cesarean section, then in his opinion there is concern if she becomes pregnant again. Therefore he wants to perform after the birth a kind of operation so that she will not be able to conceive again.
In general it is not my place to rule on halachic matters, but what is surprising here is that it is known (especially in recent years) that even after an unusual birth it is possible to have a normal delivery. Also according to doctors there is no health-based limitation on the number of such births; rather it depends on the woman's general health at the time of birth. That is, one should only decide about concern for another birth according to her health at that time. There are many treatments for maintaining general health and strengthening the mother and so forth.
Furthermore, it is known that the tzt"z ruled that in cases of concern regarding pregnancy as above he permits using a mokh (barrier), and based on his reasoning this appears less severe than an operation which completely removes the possibility of pregnancy.
Additionally (and surely this will also be explained by the doctor), usually the operation to prevent pregnancy involves tying (and sealing) the woman's tubes. Often this tying comes undone and pregnancy becomes possible again without anyone knowing. In other words, from the perspective of preventing pregnancy, one must consider carefully whether the tzt"z's method or tube-tying is better.
Another important point: In general, based on research in recent years in medical science, every day can bring new discoveries. Therefore one should avoid doing things that cannot be reversed later—even if new treatments arise that would make such actions unnecessary.
Since you write that this doctor observes Torah and mitzvos and is an expert in this field, it would be proper—after clarifying these points with another expert doctor in principle—to speak again with the Torah-observant doctor and clarify all of the above.
May Hashem Who watches over each individual with Divine Providence show you the correct path regarding these matters.
